About Midtown Health Center
Midtown Health Center (formerly Norfolk Community Health Care Clinic) has been proudly serving Northeast Nebraska since 1999. What began in Norfolk has grown to include clinics in Madison and West Point, expanding our services and team along the way. Today, we are a non-profit, 501(c)(3) Federally Qualified Health Center (FQHC), dedicated to providing compassionate, affordable care to our communities.
Our board of directors is made up of 10 committed volunteers from the region, bringing diverse expertise to guide our mission. They help ensure that we continue to deliver high-quality health services to the residents of Norfolk, Madison, West Point, and surrounding areas.
Across our four clinics, more than 95 dedicated staff members provide comprehensive care — including medical, dental, behavioral health, and specialty services.
At Midtown, you’re never just a number. As a Patient-Centered Medical Home (PCMH), we put you and your family at the heart of everything we do. We offer 24/7 access to care through our after-hours line, and our providers take the time to communicate clearly, helping you understand your health and all available options. We also connect you with community resources to support your well-being beyond the clinic walls.
